CARE COORDINATOR
The Manchester Community Resource Center, Inc (MCRCnh) is celebrating our 25th anniversary as a 501c3 nonprofit Community Based Development Organization (CBDO). With the goal of assisting low-income individuals, new Americans, people with disabilities, families receiving public assistance, individuals re-entering the community post incarceration, and/or individuals facing multiple obstacles to achieving household financial stability and other personal goals. Services are participant-focused and specifically designed to assist in improving the overall standard of living by providing access to employment, education, care coordination, information & referral, and support services in a cross-cultural environment.
The Manchester Community Resource Center, Inc (MCRCnh) is working in partnership with the State of NH Bureau of Employment Services to provide individuals that receive State cash assistance or post-TANF services with access to individualized workforce development and career advancement activities. Services are designed to assist the participants in acquiring the skill set needed to enter the workforce, to re-enter the job market, to improve employment opportunities, to attain educational credentials, to reduce obstacles by effectively utilizing community resources, and to provide financial literacy for those transitioning from public assistance with individualized workforce development activities. This position is funded 100% through a contract with the State of NH – Department of Health and Human/Division of Economic & Housing Stability Services/Bureau Employment Services (BES) – NH Employment Program.
MCRCnh is seeking to hire a highly motivated, organized, and compassionate individual to join our team as a Care Coordinator. The goal will be to complement employment related services with a complete system of interculturally competent coordination of care with wrap-around supportive services that will lead to eliminating obstacles and ensuring the progression toward upward economic mobility. The services and delivery approach of these services will be tailored to the specific needs of each individual and their family.
HIGHLIGHTS:
Coordination of Care:
Conduct assessments to identify individual skill sets, academic abilities, and obstacles.
Develop individualized service delivery plans based on assessment results and personal goals.
Assist in planning, implementing, motivating, designing, and amending service delivery plans.
Provide advocacy for access to community resources to address personal development, family strengths, community/social networks, and financial opportunities.
Offer stabilization and support services approved for NHEP Employment and Training Support (ETS) activities.
Connect participants with appropriate community services, workforce training, educational institutions, and physical/behavioral health.
Monitor and assist with state redeterminations, documentation requirements, and appointments with community partners.
Assist with obtaining copies of educational transcript, as needed.
Provide targeted care coordination tailored to individual and family needs.
Build trust and respect to create a safe learning environment for participants.
Service Delivery:
Support advanced workforce development training and services related to employment stability.
Conduct both care coordination and classroom facilitation in 1-on-1, group, in-person, and virtual settings.
Supportive services include managing household finances, career advancement, educational attainment, and wage progression.
Assist with job retention and post-employment services as needed.
Provide support with employment issues, interpersonal skills, workplace mentoring, crisis prevention and resolution, and linkage with job training.
Assist with obtaining, maintaining, and upgrading employment.
Assist with re-employment services that support former participants.
Ensure accurate communication of information regarding the pilot disregard program.
QUALIFICATIONS:
Hold a BA in psychology, sociology, or social work.
Relevant work experience or equivalent life experience may be considered.
Preferably experienced in assessment processes, care coordination, community networking, adult academic services, career development, educational enhancement services, sales, marketing, and/or other employment-related services.
Must have a working knowledge of Windows/MS Office software.
Must be willing to learn new software.
Agree to a criminal background check.
Possess a valid NH driver’s license with a clean driving record.
Demonstrate understanding of and sensitivity to the needs of the low-income population.
Ability to work with cross-cultural populations, with multilingual individuals given priority.
